Appointments

If you are unable to keep your appointment, it is necessary for you to call the office and cancel your appointment as soon as possible. We require that you call at least 24 hours in advance, and calling early in the day is appreciated. Appointments are in high demand, and your early call cancellation will allow another person the opportunity to have access to timely medical care. Late cancellations and failure to cancel will be considered a “missed appointment.” After your first and second missed appointment, our staff will call to ensure you are alright and reschedule. If you miss three consecutive appointments, our staff will call to ensure that you are alright – and then you will be sent a letter stating you may be released from the practice. After three missed appointments, you will not be given an opportunity to schedule future appointment with our practice.

How to cancel your appointment: Please call the office at 330-682-0500 between the hours of 8 a.m. - 4:30 p.m.

New Patient Forms

Please complete forms prior to arrival to office.

You will be required to present a photo ID and insurance card at each visit.

Prescriptions: All prescription refill requests should be made between the hours of 8:00a.m. - 4:00p.m. Monday through Thursday. Once the physician reviews your medical record and approves your request, your refill will be sent electronically or called into your pharmacy. Please allow 48 hours for a refill request to be completed. Prescriptions will not be called in after-hours, weekends or holidays.

Referrals: Referrals to other physicians or diagnostic facilities can take up to 72 hours for our office to process. Often prior authorization is needed by insurance companies, and it takes time to receive an authorization back.

Billing Questions: For billing questions, please call 330-479-8705.

Payment Policy:  Co-payment/co-insurance and/or account balance will be collected at each visit.

Medical Reports

Test Results: Should you have laboratory work or other diagnostic testing done through our office, you will be notified of the results as soon as they are available. All results are reviewed by the physician. You will then receive a call from the nurse, explaining your results and the need for further treatment if indicated. If you do not hear from us after one week, please call the office.

Feedback or Concerns: If you have a compliment or concern about the care you receive with Aultman Internal Medicine Center, please direct it to our Practice Administrator Lisa Casteel.
